Thank you for taking the time to fill in this Supporting People questionaire.

As you know Hartlepool Supporting People team have been producing a quarterly Newsletter for several years. In addition, over the last year we have been working with service users to produce a service user newsletter. inevitably, the information in both newsletters can be very similar and comments have been made to the team that we should have one combined newsletter with standing issue items and specific issues for providers, and also information for service users and any other interested parties.

This short questionnaire has been designed to gather opinions on how to move forward with the newsletters and their contents.

The questions are structured but there is space at the end of the questionaire to enter any other comments you may have.

Please answer all questions if possible.
