Hartlepool Support Hub

Hartlepool Support Hub – a gateway to information, advice and care

The Hartlepool Support Hub is your first point of contact for information, advice and support if you or someone you care for needs a little extra help to live well and independently.

How we can help

Our team is here to:

  • have a friendly chat to understand what matters most to you
  • help you make a personalised plan
  • connect you with local groups, activities and services
  • support you to live a good life and do the things that are important to you

We’ll work with you to find the right support, whether that’s practical help, social connection or access to care services.
