Reablement – extra help to live independently
Reablement helps you regain the skills and confidence to live independently at home. Unlike traditional home care, where someone does things for you, reablement supports you to do things for yourself.
The goal is to help you stay in your own home and avoid the need for long-term care or hospital stays.
What is reablement?
Reablement is a short-term, intensive support service. The support you receive will depend on your needs and will last for a limited time.
You do not have to pay for this service.
Who can use the service?
You can use the reablement service if you:
- live in Hartlepool and are aged 18 or over
- need support to live independently at home
- have the potential to improve
- are likely to keep using the skills you gain
- agree to take part in your reablement plan
How to get help
If you are in hospital, staff can refer you to the reablement team. Other local organisations can also make a referral.
Once we receive your referral, a reablement officer will visit you to carry out an assessment. This helps us:
- understand what you want to achieve (your outcomes)
- decide what support will help you reach those outcomes
We will then agree a plan with you. This plan will explain what you and the reablement team will do to help you meet your goals.
Your reablement officer will check your progress and update your plan regularly.
What happens after reablement?
At the end of your reablement programme, we hope you will be able to live at home with little or no support.
If you still need help, a social worker will support you to complete a self-directed assessment questionnaire (SDAQ) and create a new care plan.
