If you chose to close to our service at 21, you can ask to re‑open your support at any time until you turn 25.
You do not have to re‑open your case to receive some support. You can still use our weekday drop‑in at Pathfinders between 9am and 5pm, where staff can offer advice and guidance.
If you want full support again, you can ask to be re‑opened to Leaving Care.
When this happens, you will be given a Personal Adviser, and we will complete a Pathway Plan with you. This helps us understand your needs and how we can support you.
How to request to re‑open your support
You can ask to be re‑opened by:
- visiting Pathfinders, Villiers Street, Hartlepool, TS24 7SA
- contacting the children's services team on 01429 284284 to speak with the Children’s Hub or Leaving Care Team.
