What is a Pathway Plan?
As a care leaver, you might wonder what a Pathway Plan is and why it matters.
When you were a Child in Our Care, you had a care plan that looked at all areas of your life and how we could help. A Pathway Plan builds on this and focuses on your journey to independence.
When planning starts
Pathway planning begins at 16. Between then and your 18th birthday, the plan sets out what needs to happen to help you achieve your goals and prepare for adulthood.
Your Social Worker, Independent Reviewing Officer (IRO), Personal Advisor (PA) and other professionals will work with you to make sure you get the support you need.
Who is involved
- Before you turn 18, your Social Worker leads the plan with input from your PA and other agencies
- After you turn 18, your PA takes responsibility for reviewing and updating the plan
- Other professionals may still be involved if needed
Your involvement
The Pathway Plan is your plan. You will always be fully involved in preparing and reviewing it.
We start developing your plan within three months of your 16th birthday, and your IRO reviews it to make sure it reflects your goals.
What the plan covers
Your Pathway Plan includes support and next steps for:
- education, training and employment
- accommodation
- health and wellbeing
- family and social relationships
- finance and budgeting
- emotional and mental health
- self-care skills
- safety
- identity
We will also talk to you about any additional specific needs depending on your circumstances, for example if you are:
- at university
- on an apprenticeship
- a young parent
- leaving custody
- a separated migrant young person
- have special educational needs
How long support lasts
Once you turn 18, your PA reviews your plan with you at least every six months until you are 21. After that, you can choose to continue support up to age 25.
