Supporting your relationships

Having people you can trust and rely on is important as you move into adulthood. We want every care leaver to have a strong support network and people they can turn to for advice, encouragement and practical help when needed. Your Personal Adviser (PA) will work with you to identify the people who are important to you and help you build and maintain positive relationships.

Planning your support network

From the age of 15 years and 9 months, at your Landmark review, we begin planning for your future through your Pathway Plan. As part of this process, we will explore your support network and identify the people who may continue to support you into adulthood.

Where appropriate, we may arrange a family or support network meeting to help you identify people who can:

  • provide emotional support
  • offer practical help and advice
  • celebrate your successes with you
  • support you through difficult times

Your relationship with your Personal Adviser

Your Personal Adviser will take time to get to know you and understand what matters to you. They will work alongside you to build a strong and trusting relationship, helping you access the support and opportunities that are right for you.
