Our teams promote links across a wide range of services and work closely with children, young people and their families to build on their unique strengths, skills and resources, supporting them to be the best they can. Special educational needs or disabilities (SEND)

Support children and young people with SEND across key age ranges:

  • Early years (0–5): smooth transition into childcare and school
  • Primary (6–11): support through primary education
  • Secondary (12–16): help with transition and access to support
  • Moving On (17–25): prepare for adulthood and post-16 options

Early help roles

Our Early Help team ensures children live in safe environments. Roles include:

 

  • Health Visitors: Specialist community public health nurses (SCPHN) and registered nurses. They work with families with a child aged 0-5 years, identifying health needs early to improve health and wellbeing, prevent ill health, and reduce inequalities
  • Community Nursery Nurses (CNN): Provide vital care within the child health team, offering packages of care for families previously assessed by a health visitor
  • Family Support Workers (FSW): Take action as soon as possible to tackle issues with children and families, identifying those at risk and providing timely and effective support.
  • School Nurses: Specialist community public health nurses (SCPHN) and registered nurses. They work with school children, young people and their families to improve their health and wellbeing outcomes and reduce inequalities and vulnerabilities

Fostering and adoption

Foster carers provide temporary care for children in need. They are supported by social workers from the Fostering Team.

Youth support services

Youth Support Services offer a wide range of activities, advice and support from dedicated Youth Centres, the One Stop Shop and from various locations across the town via the Youth Service Mobile and Detached service.

The One Stop Shop offers young people a wide range of services including:

  • support into education, training or employment
  • health and relationship advice
  • support with money issues, benefits advice, student finance and budgeting
  • job search and curriculum vitae support
  • support around housing issues
  • opportunities for personal and social development
  • C Card, chlamydia and pregnancy testing

Work for Family Hubs in Hartlepool

Family Hubs are where our family support workers, family hub workers, health visitors, midwifery nurses, and community nursery nurses are based.

They support families with children aged 0 to 19 (or 0 to 25 for those with SEND), offering:

  • integrated childcare and education
  • health services
  • family support
  • access to Job Centre Plus

Family Hubs are a one-stop shop for free support, where skilled staff offer help and advice on:

  • health and parenting
  • play sessions and childcare
  • job and training support
  • meeting other parents

They also provide:

  • health visitor clinics
  • development reviews at 9–12 months and 2–2.5 years
  • guidance on feeding, toilet training, routines, safe sleeping, introducing solid foods and breastfeeding

Weekly groups include:

  • sing and sign
  • free play time
  • baby weigh-ins
  • breastfeeding support
  • story time
