What needs to be done

Review of care setting transition and timely hospital discharge through advanced care planning and coordinated care.

In line with national guidance the importance of advanced care planning, coordinated care and timely discharge will reduce unnecessary hospital deaths, reducing strain on hospital systems. Service models are recommended in national guidance to support people to die in their preferred place, often outside of hospital.

Why

There is a high rate of hospital deaths in Hartlepool in comparison to the national average, which will create a strain on hospital systems.

Strategic Issue 2

What needs to be done

Support local care homes with Palliative / End of Life Care (Elwick Grange, Sheraton Court Care Home and Merlin Manor Care Home) with the increasing demand. Particularly in planning and continuity of care for individuals entering care homes temporarily.

Why

There is a higher rate of deaths occurring in care homes in Hartlepool than nationally and the rate of deaths in care homes where it is not the usual residence is increasing. There is a need for better planning and continuity of care for individuals entering care homes temporarily. With increasing demand, care homes will require increased support and funding to meet demand.

Strategic Issue 3

What needs to be done

There is a need for targeted public health interventions for under 65s and 65-74 year olds and integrated care planning for chronic conditions and earlier palliative care involvement.

Why

Higher mortality rates in younger age groups in comparison to the national average, this is an indication of deprivation.
