What needs are unmet?
Strategic Issue 1
Due to high hospital death rates in comparison to the national average, there is a need to focus on optimal transitions between care settings and facilitating discharge from hospital to community settings through advanced care planning, coordinated care and timely discharge.
Strategic Issue 2
Due to the rising rate of care home deaths, there is a need to support local care homes with Palliative / End of Life Care (Elwick Grange, Sheraton Court Care Home and Merlin Manor Care Home) with the increasing demand. Particularly in planning and continuity of care for individuals entering care homes temporarily.
Strategic Issue 3
Due to higher mortality rates in younger age groups in comparison to the national average, this is an indication of deprivation. There is a need for targeted public health interventions for under 65s- and 65–74-year-olds and integrated care planning for chronic conditions and earlier palliative care involvement.
