What needs are unmet?

Strategic Issue 1 

Due to higher prevalence of Dementia in Hartlepool compared to regionally and nationally and the growing rate of dementia due to ageing population, there is a need for enhanced diagnostic capacity, early intervention and tailored community support.

Strategic Issue 2

The data indicates there is a gap in community-based care with rising hospital admissions for dementia and low annual review rates in primary care. This indicates missed opportunities to manage symptoms and adjust care plans.

Strategic Issue 3

There is a need to address modifiable risk factors such as physical inactivity, uncontrolled diabetes, hypertension, hearing loss and tobacco and alcohol use which are prevalent in the Borough.

Strategic Issue 4

There is a need to support care homes with providing quality end of life care and advanced care planning as the majority of patients with Dementia die within care homes in Hartlepool.
