The following downloadable documents detail the evaluation process the Council will undertake when reviewing the health and safety elements of your 'pre-qualification questionnaire' or 'suitability assessment questionnaire'. Appendix 1 - document to be used for companies with more than 5 employees involved in construction & non-construction activities deemed as high risk (excluding care)
Appendix 2 - companies involved in providing care services
Appendix 3 - document to be used for companies with less than 5 employees involved in construction & non-construction activities deemed as high risk (companies wanting to act in the capacity of designer, lead designer, principal contractor as per CDM would need relevant section of Appendix 1 evaluating even if they employ less than 5 people)
