Health inequalities and deprivation: deprivation is a major driver of prevalence and poor outcomes. Inequalities in care need to be addressed specifically around the 8 care processes and access to services.
Strategic Issue 2
The prevalence of risk factors such as obesity, physical inactivity and smoking are high.
Strategic Issue 3
Health literacy gaps exist. Of those newly diagnosed with Type 2 Diabetes (2023) that were invited for Structured Education, just 2.6% attending within 12 months of diagnosis, which is significantly lower than Type 1 Diabetes (75%).
