What needs are unmet?
Strategic Issue 1
The inequalities in care need to be addressed, specifically around the 8 care processes and access to services.
Strategic Issue 2
There is a need for targeted outreach relating to lifestyle risk factors in areas of high prevalence of diabetes, risk factors and deprivation.
Strategic Issue 3
There is a need to address digital and health literacy gaps, given the high use of digital tools for diabetic management and need for health literacy in management of the condition. Few newly diagnosed Type 2 Diabetics are attending Structured Education.
