Nationally
NHS Diabetes Prevention Programme (NHS DPP)
A joint initiative by NHS England, PHE and Diabetes UK which offers behavioural interventions to people at risk of type 2 diabetes (those with NDH). The programme focuses on weight loss, healthy eating and physical activity. It aims to reduce the annual cost of type 2 diabetes and prevent 20,000 early deaths per year.
National Diabetes Audit
Tracks delivery of the 8 care processes and 3 treatment targets.
Regionally
The North East and North Cumbria ICS is a key delivery partner for the NHS DPP. The region has higher than average diabetes prevalence, especially in areas of deprivation. Local NHS trusts and public health teams are working to: improve uptake of structured education; expand community-based prevention programmes; and address health inequalities through targeted outreach.
Locally
Community Hubs
The 4 community hubs buildings, 3 Community Connectors and 2 Lifestyle Intervention Officers provide support and information to residents.
Established weekly health walks and social groups are provided free of charge. The Community hubs are drop-in centres whereupon further 1:1 appointments are available to explore ‘what matters most’ to the individual in terms of achieving positive behaviour change. The offer from the Wellbeing team is based on the Social Prescribing model. This non-clinical approach provides the opportunity for residents to find out about all services which may be of benefit to them. The emphasis is on empowerment and to increase knowledge of the range of choices available.
The use of motivational interviewing skills assists to find the root cause of any issue for the client. Community Connectors encourage and ‘signpost’ to other professional and voluntary sector agencies; this includes a ‘buddy up’ system for those who have some level of anxiety or lack of confidence to try something new. There is no criteria around access as the Wellbeing team operate from a holistic view which enables a fully inclusive; accessible service for all.
Join the Movement
Join the Movement is a community initiative aimed at improving physical and mental health through increased physical activity. It is a network of local and regional partners looking to: address health inequalities; improve social outcomes; and promote physical and mental wellbeing.
