THE INDEPENDENT Chair of a working group created to identify priorities for the delivery of integrated health and social care services across Hartlepool will return to the town next week.

During 2015/16, Hartlepool Borough Council – working in partnership with Hartlepool and Stockton-on-Tees Clinical Commissioning Group – asked Professor David Colin-Thomé to develop a new model and his recommendations were outlined in the ‘Hartlepool Matters’ report.

On Monday 2 October, Professor Colin-Thomé will chair a special meeting of the Hartlepool Matters Implementation Plan Working Group to look at how his recommendations have been met so far, and the plans in place for the future.

A draft implementation plan has been produced to highlight what has already been achieved, what happens next and how the success of this work will be measured.

Members of the public are welcome to attend the meeting which will be held at 3pm on Monday 2 October in the Council Chamber at Hartlepool Civic Centre.
